At the Heartright Foundation, we are committed to supporting individuals with intellectual and developmental disabilities by helping ensure access to essential needs and resources. If you are currently receiving services from the Alta Regional Center or North Bay Regional Center, you may be eligible for assistance through our foundation.

Whether you are in need of monetary support, in-person assistance, or a specific item or resource, we invite you to fill out the request form below. Please include as much detail as possible about the type of assistance you are seeking, including an estimated cost if applicable.

Once your request is submitted, a member of our team will review the information and reach out to you directly if any additional details are needed. We will do our best to determine how we can support you based on your unique circumstances and our available resources.

Please note the following before submitting your request:

  • Eligibility Requirement: Assistance through the HeartRight Foundation is only available to individuals who are current clients of the Alta Regional Center or North Bay Regional Center. Unfortunately, we are unable to process requests from individuals who are not enrolled with one of these Regional Centers.

  • Application Does Not Guarantee Approval: Submitting a request for assistance does not guarantee approval. Each request is carefully reviewed by the HeartRight Foundation Board of Directors to determine how and if support can be provided based on available resources and alignment with our mission.

  • Board Review Process: All completed requests will be presented to the Board for evaluation. If your request is missing information or if further clarification is needed, a Board representative will reach out to you directly.
